- 将所有 Telegram 群链接统一更新为 t.me/polyhermes
- 在 README 和 README_EN.md 中添加动态 Docker 版本徽章，自动显示最新版本
- 更新 Layout.tsx、RELEASE.md 中的 Telegram 链接
